2) Two sxs cement to be added onto CIBP@ 7860' instead of @ 7910' (see step 9 of procedure).
3) Run CBL to verify the top of primary cement is at least 200’ over Niobrara, at least 200’ below Sussex to 200’ above Sussex, and adequately isolates the Fox Hills aquifer. If cement does not exist as required, provide this coverage as part of this plugging project. Use CBL results to verify the setting depth of and stage cement pumped through the DV tool. If DV tool cement missing, add cement behind production casing to ensure Fox Hills aquifer isolation. Add cement in casing across and 50' above and below DV tool as part of this plugging project.
4) If DV cement does not come to surface it is acceptable to cut and pull casing.
5) For 832' plug: pump plug and displace, shut-in, WOC 4 hours and tag plug – must be 159’ or shallower.
6) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.
7) Please submit existing gyro survey data with Form 6 (s) Subsequent Report of Abandonment. |

• Form 17 Bradenhead Test Report shall be submitted within 10 days of the test.
• File electronic Form 42 once flowline abandonment is complete.
• Submit Form 6 Subsequent Report of Abandonment with CBL used to verify existing coverages and DV tool placement as well as the gyro survey of 2/13/14 attached.

• Provide 48 hour notice of plugging MIRU via electronic Form 42.
• Plan is to run CBL to verify existing cement coverge and DV tool placement. Plugging procedure is designed to achieve cement coverage of at least 200' over Niobrara, 200' below the Sussex to 200' above and adequately isolate the Fox Hills aquifer from 900' to surface.
• After placing casing plug from 680-100', shut-in, wait on cement for 4 hrs and tag plug. Top of cement must be 159' or shallower.
• Place a 15 sack plug at the surface, all other plugs shall have at least 100' of cement left in the casing.
• Properly abandon flowlines as per Rule 1103.

Prior to starting plugging operations, a bradenhead test shall be performed. If the initial
shut-in bradenhead pressure is greater than 25 psi, notify COGCC Engineer for
sampling requirements prior to beginning the test. Sampling shall comply with Operator
Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and
Gas Sampling. |
