1) Run CBL to verify the top of primary or squeezed cement is at least 200’ over Niobrara, 200' below and 200' above Sussex (productive within 1 mile) , and adequately isolates the Fox Hills aquifer. If it does not exist as required, provide this coverage as part of this remedial cementing project. 2) The additional cement referenced shall be placed as indicated and comply with Rule 317.j. The placed cement shall be verified with a CBL and documented with a Form 5 Drilling Completion Report. 3) Please submit gyro survey data with Form 5 Drilling Completion Report.

1) Prior to starting plugging operations, a bradenhead test shall be performed. If any pressure remains at the conclusion of the test or any liquids were present contact COGCC Engineer for sampling requirements. The Form 17 shall be submitted within 10 days of the test. 2) Provide 48 hour notice of plugging MIRU via electronic Form 42. 3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 4) Sussex plug must be tagged at 4250' or shallower. Production casing stub plug is also surface casing shoe plug, this plug must be tagged at 649’ or shallower. Leave at least 100’ cement in the wellbore for each plug.
