Open hole resistivity and gamma logs shall be run to describe the stratigraphy of the entire well bore and to adequately verify the setting depth of surface casing and aquifer coverage. On a multi-well pad, these open hole logs are only required on one of the first wells drilled on the pad and the Drilling Completion Report - Form 5 for every well on the pad shall identify which well was logged.

1. Daily drilling reports are required. 2. A static water level taken after perforation in cased wells and prior to any stimulation in either cased or open-hole wells. 3. Retrieve water sample(s) from injection zone(s) before stimulating formation, Step Rate Test, or Injectivity Test. 4. A Transient Injectivity Test is required to determine the Maximum Daily Injection Rate in barrels per day (bbls/day). 5. A Transient Injectivity Test or a Step Rate Test is required to determine the Maximum Injection Pressure in pounds per square inch (psi). 6. Prior approval of Form 4 is required for Step Rate and Injectivity Tests. 7. Injection is not authorized until approval of Forms 31 and 33.
