(3) INJECTION IS NOT AUTHORIZED UNTIL APPROVAL OF FORMs 31 AND 33. (4) A STEP RATE TEST TO DETERMINE FRACTURE PRESSURE IS PERMISSIBLE BEFORE APPROVING FORMS 31 AND 33, BUT MUST BE GIVEN COGCC APPROVAL PRIOR TO CONDUCTing THE TEST. THE TEST VOLUME MUST NOT EXCEED 5,000 BBLS AND THE TEST DURATION MUST NOT EXCEED 5 DAYS, OR THE TEST VOLUME MUST NOT EXCEED 10,000 BBLS AND THE DURATION MUST NOT EXCEED 10 DAYS.

1) Submit documentation of repair with a wellbore diagram that meets COGCC wellbore diagram guidelines and includes formation tops. Required formation tops are: Wasatch G, Lower Wasatch, Ohio Creek, Williams Fork, and Rollins.

Well must pass an MIT prior to being returned to service. Current maximum permitted surface injection pressure is 647 psi. If operator wishes to inject at or near this pressure, as they have done in the past, the MIT test pressure should be about 950 psi per Page 5 of COGCC's Mechanical Integrity Test Guidance: Practices and Procedures. The object of the MIT is to run the test with 300 psi above the intended injection pressure.
