1. Operations may begin as early as August 12, 2024, with a 4-6 week timeline.
2. Operations should only be active Monday through Friday; weekend work may be
completed if there is an emergency situation.
3. Precautions to avoid our North Restroom leach field must be taken.
4. Equipment should be cleaned to avoid invasive weed spread prior to it being
brought to the work site.
5. Any vegetation and grasses disturbed by the project must be reseeded with a Barr
Lake native grass seed mix following completion of the project. We will provide you
with the mix.
6. Any state property damaged or destroyed due to this project must be repaired or
replaced to like-for-like or better condition.
7. A certificate of insurance document listing Farmer's Irrigation Company and Barr
Lake State Park as additional insured must be provided. Insurance minimum must
be $1,000,000.
8. If any traffic delays or short duration closures (5 minutes or less) are needed, I need
to be notified in advance and you must provide flaggers to direct traffic.

| Engineer | Form: (06) 403898614 09/25/2024 | 1) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations and electronic Form 42 Notice of Plugging Operations 48 hours prior to mobilizing for plugging operations. These are two separate notifications, required by Rules 405.e and 405.l.
2) Pump surface casing shoe plug at 272' only after isolation has been verified. If surface casing cement is not circulated to surface, shut-in, WOC 4 hours then tag plug – must be at 46'’ or shallower and provide a minimum of 10 sx plug at the surface.
3) Leave at least 100’ of cement in the wellbore for each plug without mechanical isolation.
4) After cut and prior to cap, verify isolation by either a 15 minute bubble test or 15 minute optical gas imaging recording. If there is indication of flow contact ECMC Engineering. Provide a statement on the 6SRA which method was used and what was observed. Retain records of final isolation test for 5 years.
5) With the Form 6 SRA operator must provide written documentation which positively affirms each COA listed above has been addressed. |
