1) Provide 24 hr notice of spud to Bo Brown at 970-330-6085 or email at bo.brown@state.co.us . 2) Set surface casing per Rule 317d, cement to surface. Setting surface casing less than the approved depth is a permit violation unless prior written approval is obtained from the COGCC. 3) If completed, provide cement coverage from TD to a minimum of 200' above Niobrara. Verify coverage with cement bond log. 4) Comply with Rule 321. Run and submit Directional Survey from TD to base of surface casing. Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.

Prior to starting plugging operations a bradenhead test shall be performed. 1)If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2)If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. 3)If sampling is required contact COGCC engineering for a confirmation of plugging requirements prior to placing any plugs. 4)Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. The Form 17 shall be submitted within 10 days of the test. 5)Submit Form 42 electronically to COGCC 48 hours prior to MIRU 6)Prior to cutting casing or placing the 1050’ plug: verify that all fluid migration (liquid or gas) has been eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer for an update to plugging requirements. 7)After isolation has been verified, pump plug and displace. If cement is not circulated to surface, shut-in, WOC 4 hours and tag plug – top of plug must be not deeper than 704’ and provide minimum 10 sx plug at the surface. 8)Leave at least 100’ of cement in the wellbore for each plug. 9)Properly abandon all flowlines. Once flowlines are properly abandoned, file electronic form 42.
