2. Prior to starting bradenhead abatement, if a sample has not been collected within the last twelve months collect bradenhead and production gas samples for laboratory analysis. Sampling will comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results will be provided to the COGCC within three months of collecting the samples.
3. Bradenhead gas is not to be vented to atmosphere; any gas from the Bradenhead will be routed to an enclosed tank and/or combustor. Operator will implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. The abatement program may be used for six consecutive months.
4. Operator will submit a Form 42 (“OTHER”) stating that “bradenhead abatement program has started.”
5. At the conclusion of the six months, conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans and the flow rate information and pressure data.

2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples.

1) Provide 2 business days notice of plugging MIRU via electronic Form 42.
2) After placing the shallowest hydrocarbon isolating plug (1550'), operator must wait a sufficient time to confirm static conditions.
3) After placing plug at 1550’ assure that all fluid migration has been eliminated by monitoring the well for a minimum of 8 hours before proceeding to the next plug. If at any time after placing this plug there is evidence of pressure or of fluid migration, contact COGCC engineering before continuing operations.
4) Prior to placing the 815’ plug: verify that all fluid migration (liquid or gas) has been eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer for an update to plugging orders.
5) After isolation has been verified, pump plug and displace. If cement is not circulated to surface, shut-in, WOC 4 hours then tag plug – must be at 563’ or shallower and provide at least 10 sx plug at the surface.
6) Leave at least 100’ of cement in the wellbore for each plug.
7) Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment is complete. Within 90 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line, the operator must submit a Flowline Report, Form 44.
8) With the Form 6 SRA operator must provide written documentation which positively affirms each COA has been addressed.

| OGLA | Form: (06) 402607971 03/05/2021 | 1. Operator will provide written notification to the Surface Owner of plugging operations not less than thirty (30) days prior to initiating plugging operations and pursuant to Rule 309.b.
2. Operator will provide written notice to residential building unit (RBU) owners/occupants within 1000 feet of the location a minimum of 30 days prior to initiating plugging operations. Notice will be provided in both English and Spanish, delivered by hand, certified mail, return-receipt requested, or by other delivery service with receipt confirmation.
3. Operator will post signs at the intersection of the access road and Weld County Road 5050 to notify area residents of plugging operations. The sign(s) will be no less than two (2) square feet, and no more than six (6) square feet, and will provide: the name of the operator; a phone number at which the operator can be reached at all times; a phone number for local emergency services (911 where available); the Oil and Gas Location name; the legal location, including quarter-quarter section and the assigned address.
4. Operator will conduct plugging and abandonment of multiple wells on the Dickens multi-well pad during the same occupation.
5. Operator will comply with all applicable Weld County Code Ordinance 2020-12 requirements as the relevant local government, pursuant to plugging and abandonment operations.
6. Operator will properly cover, store, and manage all materials and chemicals within containment for plugging and abandonment operations brought on location to prevent any release and minimize adverse impacts to surface water or groundwater.
7. If not already in-place, sufficient storm water containment and erosion control measures will be implemented to ensure that any fine sediment or other impacted runoff is not entering the Union Ditch adjacent to the location.
8. Operator will develop a noise mitigation plan for the plugging operations to comply with daytime noise levels for Residential/Rural areas. For periodic, impulsive, or shrill noise within 1000 feet of a residential building unit (RBU), Operator will minimize noise that can be readily eliminated through maintenance, equipment modification, or other readily available procedures and will implement noise controls to protect RBUs to the fullest extent possible. The plan may require use of temporary portable sound walls or stacked straw bales to shield RBU located within 1000 feet of location if noise cannot be minimized or mitigated by other measures to comply with daytime noise levels.
9. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare. This will include blowing down the well to a tank(s) connected to an enclosed combustion device (ECD).
10. Operator will develop an emergency action plan and will coordinate with the local emergency agencies (Police/Fire Department) a minimum of 14-days prior to the initiation of the plugging operations.
