Reporting • Form 17 Bradenhead Test Report shall be submitted within 10 days of the test. • File electronic Form 42 once flowline abandonment is complete. • Submit gyro survey run on 9/2/14 with Form 6 SRA.

Plugging • Provide 48 hour notice of plugging MIRU via electronic Form 42. • Production casing stub plug is surface casing shoe plug as well. If it is not circulated to the surface then tag the plug – must be 586' or shallower. • If, after setting the bridge plug with 2 sx at 4055', there is fluid migration or shut-in well head pressure, contact COGCC Engineer for revised plugging plans before cutting casing at 1000'. • Place a 25 sack plug at the surface, all other plugs shall have at least 100' of cement left in the casing. • Properly abandon flowlines as per Rule 1103.

Bradenhead Prior to starting plugging operations, a bradenhead test shall be performed. If the initial shut-in bradenhead pressure is greater than 25 psi or liquids are present at any time, notify COGCC Engineer for sampling requirements prior to proceeding with the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.
