Please clean up the production reporting. March 2009 - Dec 2010 reports Nio- Codell and Codell. Sept 2015-present, only the Codell is reported and the Niobrara drops off.

Prior to starting plugging operations a bradenhead test shall be performed. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. The Form 17 shall be submitted within 10 days of the test.

1) Provide 48 hour notice of plugging MIRU via electronic Form 42. 2) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 3) Please submit gyro survey data with Form 6 Subsequent Report of Abandonment. 4) Production casing discrepancies on completion reports and wellbore diagrams previously submitted. Production casing is reported as 4.5-inch and 5.5-inch. If production casing is found to be 5.5-inch cement volumes must be increased to obtain the planned plug size. 5) No CBL on file with COGCC. Verify existing cement with a CBL prior to plugging and submit CBL with Form 6 Subsequent Report of Abandonment. If cement is not present on CBL as outlined on this Form contact COGCC engineer for an update to plugging orders. 6) Production casing stub plug is also surface casing shoe plug, this plug must be tagged at 497’ or shallower. Leave at least 100’ of cement in the wellbore for each plug.
