3) Complete a Form 21 MIT to verify repairs for injection wells. MIT is required to be approved prior to returning well to injection
4) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations
5) Operator shall notify ECMC Engineering staff of changes and/or additional repair methods to the approved repair procedure. |
