9/17/2015 | 1.Injection is not authorized until approval of Forms 31 and 33.
2.Operator required to contact COGCC to discuss Step Rate Test or Injectivity Test criteria for Maximum Surface Injection Pressure determination.
3.Prior approval of Form 4 is required for step rate and injectivity tests.
4.Retrieve water sample(s) from injection zone(s) before stimulating formation. Samples must be analyzed for Total Dissolved Solids at a minimum.
5.Operator must provide all tops of formations encountered from surface to TD on the Form 5 when submitted.

9/25/2015 | 1. Provide remedial cement from 550' to surface (at least 50' above surface casing shoe). WW's w/in 1 mile and 2 mi. are 320' and 460' deep.
2. Run CBL on the 5.5" casing prior to running the 4.5" casing. Verify all cemented intervals and TOC at least 200' above D-Sand.
3. Submit Form 5 - Drilling Completion Report. Update top of primary cement and all other cement intervals behind 5.5" casing, and add the 4.5" casing. From 1994 plugging reports there were two 25 sk cement plugs set at 4398' and 2154', across casing leaks 4340-4398' and 1980-2110'. There were also possible casing leaks identified on 1991 doc#250652 repair report/downhole schematic at 3741-4300'. |
