3) If well is a dry hole set plugs at the following depths: 40 sks cement +/- 50’ above the MRRW, 40 sks cement +/- 50’ above the CHRK, 40 sks cement +/- 50’ above the MRTN, 40 sks cement +/- 50’ above the LNSG, 40 sks cement across any DST w/ show, 40 sks cement 50’ below Cheyenne (4600’ up), 40 sks cement 50’ above top of Dakota (4000’ up), 50 sks cement from 50’ below surface casing shoe up into surface casing, 15 sks cement in top of surface casing, cut 4 ft below GL, weld on plate, 5 sks cement each in rat hole and mouse hole.

1) Provide 48 hour notice of MIRU via electronic Form 42.
2) Shoe plug (350'): Tag plug 50’ above surface casing shoe.
3) Surface plug: Cement from 50' to surface in casing and annulus.
Bradenhead:
1) If there has not been a reported Bradenhead test within 60 days of plugging this well, prior to starting plugging operations a Bradenhead test shall be performed. The Form 17 shall be submitted within 10 days of the test.
2) If before opening the Bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
3) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.
Venting during plugging:
Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.
Flowlines:
Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment is complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line, the operator must submit a Flowline Report, Form 44.
