Operator is to monitor the bradenhead and notify COGCC engineering staff if the pressure exceeds 0.25 times the surface casing setting depth (TVD) in psi.
2)Operator is to perform a bradenhead test and submit the results on a Form 17 within one month of the procedure completion.
3)Submit subsequent Form 5 at the completion of this procedure including a wellbore diagram per COGCC wellbore diagram guidelines.
