<p><span class="Normal">Prior to recompletion, operator must: 1) Provide 24 hour notice of MIRU to Jim Precup at 303-469-1902 or e-</span><span class="Normal">mail at jim.precup@state.co.us. 2) Place remedial cement to comply with Rule 317.i, 50' below Fox-Hill to 50' </span><span class="Normal">above Fox-Hill (minimum coverage 160’ to 1150'). 3) Cement coverage shall be provided from TD to a </span><span class="Normal">minimum of 200' above Niobrara, along with 200’ above and below Sussex. 4) Verify cement coverage with a </span><span class="Normal">cement bond log. 5) Upon well recompletion operator shall file a COGCC Form 5, Completion Report </span><span class="Normal">reflecting the actual casing and cement configuration.</span></p>

2) Leave at least 100’ of cement in the wellbore for each plug.
3) Properly abandon flowlines. Once flowlines are properly abandoned, file electronic form 42.
