Well name: Government 23-1 Date: 1/3/2013 Field: Ryan Gulch API: 05-103-07355 Surface Casing: 8 5/8" 24# set @ 450-ft Production Casing: 5-1/2" 14# @ 4,928-ft Completions: 4,688'-4,864' Purpose: Repair Hole in Production Casing Proposed Procedure: 1) Run CBL 2) MIRU Cementing Service Company 3) Conduct safety meeting and pressure test lines 4) Mix 15.8 #/gal cement 5) Pump Cement Balanced Plug over this interval extending 200' above Hole 5a) Small Hole between 3,960' - 3,992' 5) Shoot Holes at 2,618' (100' below Wasatch top) 6) Set Retainer at 2,550' 7) Squeeze 150 sacks through retainer, sting out and dump last two sacks on retainer 8) Circulate cement down casing through hole between 876'-891' and circulate to surface 9) Cut off wellhead 10) Top off Cement 11) Install metal plate at least ½” thick and welded in place (metal plate needs a weep hole) The following needs to be on the cap: 11a) Operator Name 11b) Federal Lease Serial Number 11c) Well Number 11d) Location by 1/4 1/4 , Section, Township, and Range Mark, Consider this your verbal approval to proceed with the P&A. WPX needs to submit a Form 6 Notice of Intent to Abandon no later than Monday next week with this procedure for formal approval. A Form 6 Subsequent Report of Abandonment is also required with cement tickets within 30 days after the work is complete.
