After making-up three joints of 5 1/2" production casing, rig driller inadvertantly dropped three joints of casing in the hole. The total fish length was 128'. A BHA was ran in the hole and tagged the fish at 3,320' MD. A fishing assembly was later ran in the hole, tagged the fish at 3,320' and pushed the fish to 3,520' MD. At, 3,520' MD, the fishing assembly became stuck. After several hours of working the string, the string became loose and was pulled out of the hole. The fish was not retrieved form the hole and it was decided to terminate fishing operations and proceed with sidetracking the well. An Applicaiton for Permit to Drill (Sidetrack) (Form 2) was submitted on April 23, 2011 as well as a Sundry Notive (Form 4). This Drilling and Completion Report (form 5) is for the original wellbore. Conductor was cemented with 7 cubic yards of redi-mix. Had 8 bbls of cement back to surface on Surface casing cementing.
