Description of Well Site and Construction Area
1-3 acre site with access road for drilling of oil and gas well. Terrain is rolling with native grass and sagebrush. Surface Roughening will be used on low side of drill site to reduce speed of runoff, increase infiltration, reduce erosion, trap sediment and prepare soil for reseeding. Drilling pits closed and reclaimed within 3 months weather permitting. Topsoil will be separated and spread on drill site as final operation before reseeding operations which will be completed within 6 months weather permitting. To prevent soil erosion site will be disked and straw crimped in or manure spread. Barb wire fence 292' West of well pad will not be disturbed by drilling operations.

Description of Well Site and Construction Area. 1-3 acre site with access road for drilling of oil and gas well. The Ma-State # 12S is a proposed second vertical well to be drilled 147' south of the producing Ma-State 8S oil well within the original 3 acre disturbed area. The drilling pit will also be within original disturbed area for Ma-State 8S. Terrain is flat with native grass and sagebrush. Surface Roughening will be used on low side of drill site to reduce speed of runoff, increase infiltration, reduce erosion, trap sediment and prepare soil for reseeding. Barb wire fence 280' west of well pad and shallow surface water depression which is dry located 1,615' south from well stake, both sites will not be disturbed by drilling operations. Drilling pits closed and reclaimed within 3 months weather permitting. To prevent soil erosion site will be disked and manure spread. Topsoil will be separated and spread on drill site as final operation before reseeding operations which will be completed within 6 months weather permitting.
