Submit a Site Investigation Workplan (Form 27) by September 1, 2019, to verify that a hole in the surface casing of the District Six C6 (123-2411) well did not cause an impact to soil or groundwater. At a minimum, include a plan to perform a soil gas survey of the entire location at each well on the pad, including all wells that were plugged. Include a plan to install a monitor well to assess shallow groundwater quality adjacent to the District Six C6 well that was previously plugged. The monitor well shall be installed to at least 20-feet deeper than the depth of the casing hole. Provide a proposed monitor well location and construction diagram with the Form 27.

If such meeting should occur, the City of Greeley will help organize the meeting through the Use by Special Review process.

The Noise Study conducted by consultants Behrens and Associates, Inc. for Greeley Directional is in compliance with COGCC Section 802 “Noise Abatement”. The site is bordered by industrial zoned property and residential to the northwest and west. No additional mitigation is required for this site of using sound mufflers or sound walls. The Unmitigated noise impact models representing drilling and fracing operations at the site were constructed and the results compared to the COGCC Standard.

The following Best Management Practices will be implemented during MLVT installation: The site shall be prepared in accordance with tank manufacturers specification prior to tank installation; including ensuring that proper compaction requirements have been met. A 36-MIL fabric reinforced liner will be utilized. In the event that a tank breach were to occur, the fabric reinforced liner will prevent a “zippering” failure from occurring.
