3/29/2018 | 1. Injection is not authorized until approval of SUBSEQUENT Forms 31 and 33.
2. Retrieve water sample(s) from injection zone(s) before stimulating formation. Samples must be analyzed for Total Dissolved Solids at a minimum.
3. PRIOR TO PERFORMING OPERATIONS: Operator is required to A. contact COGCC to discuss and B. file a Form 4-Sundry Notice to document design of proposed Step Rate or Injectivity Test for Maximum Surface Injection Pressure determination.
4. PRIOR TO PERFORMING OPERATIONS: Operator is required to A. contact COGCC to discuss and B. file a Form 4-Sundry Notice to document any proposed Hydraulic Fracturing and Acidizing jobs.
5. An MIT with tubing set in final configuration for injection is required for approval of SUBSEQUENT Forms 31 and 33.
6. For all new and converted Underground Injection Control wells a Cement Bond Log (CBL) is required on the cased portions of the hole from the bottom of the casing to the top of the next shallower casing string for all casing strings other than the Surface Casing. Only a PDF, TIFF, or PDS visual image is required. CBL required for 1st String Casing.
7.Operator must provide all tops of formations encountered from surface to TD on the Form 5 when submitted for new wells or Form 4 Sundry Notice for existing well conversions. |

4/26/2018 | New MIT must be performed after final tubing and packer have been installed not before. Operator may perform other pressure tests to check integrity but final MIT for injection permit must be with well in final injection configuration. Final MIT must be witnessed by COGCC.
