| UIC | Form: (31) 400970835 07/18/2016 | Form 26 required. |
| UIC | Form: (31) 400970835 07/18/2016 | 1. Injection is not authorized until approval of SUBSEQUENT Forms 31 and 33.
2. PRIOR TO PERFORMING OPERATIONS: Operator is required to A. contact COGCC to discuss and B. file a Form 4-Sundry Notice to document design of proposed Step Rate or Injectivity Test for Maximum Surface Injection Pressure determination.
3. PRIOR TO PERFORMING OPERATIONS: Operator is required to A. contact COGCC to discuss and B. file a Form 4-Sundry Notice to document proposed Hydraulic Fracturing and Acidizing job.
4. Retrieve water sample(s) from injection zone(s) before stimulating formation. Samples must be analyzed for Total Dissolved Solids at a minimum.
5. MIT with tubing set in final configuration for injection required for approval of SUBSEQUENT Forms 31 and 33. |
| UIC | Form: (31) 400970835 07/18/2016 | Maximum Surface Injection Pressure and Maximum Injection Volume to be determined via Subsequent Form 31/33 submittals. |
| UIC | Form: (31) 401125575 04/05/2019 | Within thirty (30) days of initial injection Caerus shall submit a Form 5A Completed Interval Report notifying the COGCC of the date of initial injection and updating the status of the Ohio Creek Formation. |
| UIC | Form: (31) 401125575 04/05/2019 | The maximum authorized daily injection rate shall be 8,000 bbls/day. Caerus must confer with COGCC if in the future it wishes to increase the daily injection rate above 8,000 bbls/day. |
| UIC | Form: (31) 401125575 04/05/2019 | The Savage 8A-794-SWD well is currently permitted to inject fluids from wells listed on Form 26-Source of Produced Water for Disposal (Document 401129140) approved April 26, 2017. Sources of produced water and other approved Class II fluids shall be added or deleted by submitting supplemental Form 26’s. |
| UIC | Form: (31) 401125575 04/05/2019 | The nature of the injected fluids shall be monitored with sufficient frequency to yield data representative of their characteristics. Therefore, as a condition of approval a water analysis of fluids injected into the Savage 8A-794-SWD is required within one year of the commencement of injection. The injected water must be analyzed for total dissolved solids (“TDS”), major cations, and major anions. This analytical data set must be submitted to COGCC via a Form 4-Sundry Notice and may be submitted via COGCC’s Electronic Data Deliverable (“EDD”) Format. Additional analyses, as described above, of the fluids injected into the Savage 8A-794-SWD well are required at 5-year intervals. |
| UIC | Form: (31) 401125575 04/05/2019 | In accordance with COGCC Rule 326.a.(5), mechanical integrity tests (“MIT’s”) shall be performed at 5-year intervals on the Savage 8A-794-SWD well, as long as it is used for the injection of fluids. The first 5-year period shall commence on the date the initial mechanical integrity test is performed, July 25, 2017 in this case. A MIT is also required after resetting the tubing or packer whenever the tubing or packer is disturbed during workover operations. All injection well MIT’s must be witnessed by COGCC Staff. |
| UIC | Form: (31) 401125575 04/05/2019 | Other Class II fluids (i.e. NOT including produced water, used drilling fluids, used workover fluids, used stimulation fluids, and used fluids from circulation during cementing operations) must be approved on Form 14A (Authorization of Source of Class II Waste for Disposal) by COGCC Staff prior to injection. These fluids must be reported on Form 14 (Monthly Report of Non-Produced Water Injected).
