Verify coverage with cement bond log. 4) Run and submit Directional Survey from TD to base of surface casing. Form 17, 402261845, from 12/12/19 reported bradenhead pressure of 131 psi that blew down to a whisper and that samples were taken. To date the analytical results are not appearing in the COGCC database. Submit results via Form 43 by 7/22/2020.

1. Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. Prior to starting bradenhead mitigation, if a sample has not been collected within the last twelve months collect bradenhead and production gas samples for laboratory analysis. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results shall be provided to the COGCC within three months of collecting the samples. 3. Operator shall implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up. This mitigation plan may be used for six consecutive months from the date test reported on the Form 17. 4. At the conclusion of the six months (12/5/2020), conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans and the flow rate information and pressure data. Shut in bradenhead pressure shall not exceed 50 psig.

12/12/19 FOrm 17 - SCP=131, blew down to 1 psi, samples=g 6/5/20 Form 4 - 6 months of blowdowns 12/3/20 Form 17 - SCP=114 psi, blew down to 0 psi, samples=g Since this sundry was submitted another Bradenhead test has been performed and submitted. SCP=95 psi, blew down to 0 psi, samples=g. Bradenhead pressure is not below the action threshold. Operator will perform a Bradenhead test 30 days from approval of this sundry and submit a Form 17 within 10 days along with a sundry containing a new Pressure Measurement Plan for this well.

Most recent Bradenhead testing for this well shows the SCP to be greater than the action threshold. Operator will submit a sundry within 30 days of the approval of this form to provide an update on the state of the tie-in referenced in this current submission.

Stormwater management plans (SWMP) are in place to address construction, drilling and operations associated with oil and gas development throughout the State of Colorado. BMPs will be constructed as necessary to prevent stormwater from leaving the construction site. BMPs used will vary according to the location, and will remain until the pad is reclaimed.

Spill Prevention Control and Countermeasures (SPCC) plans are in place to address any possible spill associated with oil and gas operations throughout the State of Colorado. • Materials and fluids will be stored in a neat and orderly fashion. • Waste will be collected regularly and disposed of at an offsite facility. • Prompt cleanup is required of spills to minimize waste materials entering the stormwater runoff. • Drip pans will be used during fueling and maintenance to contain spills or leaks. • Cleanup of trash and discarded material will be done at the end of the work day. • Cleanup will consist of monitoring the road, location and any other work areas. • Material to be cleaned up includes trash, scrap, and contaminated soil.

One of the first wells drilled on the pad will be logged with an open-hole resistivity log with gamma-ray from TD to into the surface casing. All wells on the pad will have a cement bond log with gamma-ray run on the production casing (or intermediate casing if production liner is run). All wells on the pad will have the horizontal portion of the wellbore logged with a measured-while-drilling log with gamma-ray. The Form 5, Completion Report, for each well on the pad will list all logs run and have those logs attached. The Form 5 for a well without open-hole logs shall clearly state “No open-hole logs were run.” The Form 5 for a well without open-hole logs shall clearly identify (by API#, well name &number) the well in which open-hole logs were run.
