Drilling/Completion Operations | Form: (02 ) 400373999 2/28/2013 | Prior to drilling operations, Operator may perform an anti-collision review of existing offset wells that have the potential of being within close proximity of the proposed well. This anti-collision review may include MWD or gyro surveys and surface locations of the offset wells with included error of uncertainty per survey instrument, and compared against the proposed wellpath with its respective error of uncertainty. If current surveys do not exist for the offset wells, Operator may have gyro surveys conducted to verify bottomhole location. The proposed well may only be drilled if the anti-collision review results indicate that the risk of collision is sufficiently low as defined by the anti-collision plan, with separation factors greater than 1.5, or if the risk of collision has been mitigated through other means including shutting in wells, plugging wells, increased drilling fluid in the event of lost returns or as is appropriate for the specific situation. In the event of an increased risk of collision, that risk will be mitigated to prevent harm to people, the environment or property. For the proposed well, upon conclusion of drilling operations, an as-constructed directional survey will be submitted to COGCC with the Form 5. |
Drilling/Completion Operations | Form: (02 ) 400373999 2/28/2013 | Kerr McGee acknowledges and will comply with the COGCC Policy for Bradenhead Monitoring during Hydraulic Fracturing Treatments in the Greater Wattenberg Area dated May 29, 2012. |
Community Outreach and Notification | Form: (04 ) 403018443 5/4/2022 | Courtesy notifications will be sent to all parcel owners with building units within 1,500 feet of the location letting them know about our subsequent well operations. Signs will be placed in conspicuous locations and will include Oxy contact information; the Oxy Stakeholder Relations team will respond to all community member inquiries. The signs will be placed so as not to create a potential traffic hazard |
Planning | Form: (04 ) 403229667 1/3/2023 | Operations are daylight-only; no lighting impacts are anticipated from operations. |
Community Outreach and Notification | Form: (04 ) 403229667 1/3/2023 | Courtesy notifications will be sent to all parcel owners with building units within 1,500 feet of the location letting them know about our subsequent well operations and providing contact information for Kerr McGee’s response line and online resources. |

Wildlife | Form: (04 ) 403229667 1/3/2023 | This planned location is located within a native aquatic species buffer. KMOG will implement stormwater BMPs (Including the use of straw wattle) during operations to prevent sediment from leaving the location. |
Dust control | Form: (04 ) 403229667 1/3/2023 | Water will be placed on dirt access roads to mitigate dust as needed. |
Noise mitigation | Form: (04 ) 403229667 1/3/2023 | Operations will be in compliance with Table 423-1 requirements. Based off the rig sound signature, rig orientation will be considered to reduce noise levels to nearby building units. |
