1) Provide 24 hr notice of MIRU to Colby Horton at 970-370-0040 or colby.horton@state.co.us.
2) Set surface casing per Rule 317d, cement to surface. Setting surface casing less than the approved depth is a permit violation unless prior written approval is obtained from the COGCC.
3) If completed, provide cement coverage from the intermediate casing shoe to a minimum of 200' above Niobrara. Verify cement with a cement bond log.
4) Comply with Rule 321. Run and submit Directional Survey from TD to base of surface casing. Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

2) Shoe plug: Run CBL to confirm TOC at 970'. Adjust procedure accordingly to achieve cement plugs in 7" casing and in 7"x 9.625" annulus at: 1400'-1060' (from 100' below Upper Pierre aquifer to 50' above surface casing shoe), 600'-500' (isolates between U. Pierre aquifer and base of Fox Hills).
3) Surface plug: Cement from 50' to surface in casing and annulus.
4) Properly abandon flowlines per Rule 1103. File Form 42 when done.
5) Abandoned well marker shall be inscribed with the well identification per Rule 319.a.(5).

Prior to initiation of plugging operations, a Bradenhead test shall be performed. Form 17 shall be submitted within 10 days. If there is 25 psi or greater pressure on the Bradenhead, or flowed any liquids from the Bradenhead, collect samples.
