Verify coverage with cement bond log. 3) Run and submit Directional Survey from TD to base of surface casing. The operator shall comply with Rule 321, and it shall be the operator’s responsibility to ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well. |

3/12/2024 | 1) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations.
2) The additional cement referenced shall be placed as indicated and codumented with a Form 5 within 30 days of repair operations. The placed cement shall be verified with a CBL. Repair shall also be pressure tested for a minimum of 15 minutes. Pressure test and CBL shall be provided with the Form 5. |
