1/23/2013 | 1) Provide 48-hour notice of MIRU to Jim Precup at (303) 726-3822.
2) Provide an “As Plugged” wellbore diagram identifying the specific plugging completed.
3) Cement isolation is required in both the casing and annular space 1) from the TOC (6830’) to 200’ above the Niobrara formation (6652’) and 2) from 950’ to the bottom of the angel pumped at 738’. Please see the revisions to the plugging orders to add these plugs.
4) For the surface casing shoe plug, if not circulated to surface, tag plug, it must be at least 150’ or higher, and a surface plug must be provided.
5) Properly abandon flowlines as per Rule 1103.
