12/26/2012 | 1) Provide 48 hour notice of MIRU to Jim Precup at (303) 726-3822.
2) Provide an “As Plugged” wellbore diagram identifying the specific plugging completed.
3) Operator shall provide the CBL referenced and the cement job summary documenting the cement squeeze performed on 10/23/2012 with the Form 6 Subsequent Report of Abandonment.
4) For the surface casing shoe plug, if not circulated to surface, tag plug, it must be at least 195’ or higher, and a surface plug filling all remaining casing strings must be provided.
5) Properly abandon flowlines as per Rule 1103.
