9/8/2010 | Prior to recompletion, operator must:
1) Provide 24 hour notice of MIRU to Bo Brown at 970-330-6085 or email at bo.brown@state.co.us.
2) Place remedial cement to comply with Rule 317.i, 50' below Fox-Hill to 50' above surface casing shoe (minimum coverage 175’ to 340').
3) Verify new cement coverage and that cement is 200’ above top of Niobrara with a cement bond log.
4) Upon well recompletion operator shall file a COGCC Form 5, Completion Report reflecting the actual casing and cement configuration. |

8/27/2015 | Note changes to plugging procedure:
1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) For 1080’ plug: pump plug and displace. If surface casing plug is not circulated to surface then tag plug – must be 123’ or shallower and provide 10 sx plug at the surface. Leave at least 100’ cement in the casing for each plug.
3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.
