2) Submit a Form 5a to report the packer setting depth.
3) Contact ECMC Area Engineer if scope of repairs changes from what has been proposed herein.
4) Well is to remain disconnected from injection effective upon the failed MIT of 4/15/2024. Plug or repair and return to injection only after demonstrating mechanical integrity with a satisfactory, witnessed Mechanical Integrity Test by 10/15/2024. |
