Engineer | Form: (02 ) 401638301 6/6/2018 | 1)Operator shall comply with the most current revision of the Northwest Notification Policy. |
UIC | Form: (02 ) 401638301 6/22/2018 | 1. Injection is not authorized until approval of Subsequent Forms 31 and 33.
2. Retrieve water sample(s) from injection zone(s) before stimulating formation. Samples must be analyzed for Total Dissolved Solids at a minimum.
3. PRIOR TO PERFORMING OPERATIONS: Operator is required to contact COGCC to discuss acid and fracturing jobs. Approval of Form 4 may be required for acid and fracturing jobs. (As of 4/13/2016)
4. PRIOR TO PERFORMING OPERATIONS: Operator is required to contact COGCC to discuss Step Rate Test or Injectivity Test criteria for Maximum Surface Injection Pressure determination. Prior approval of Form 4 may be required for step rate and injectivity tests.
5. Operator must provide all tops of formations encountered from surface to TD on the Form 5 when submitted. |
UIC | Form: (04 ) 401745514 9/4/2018 | Job is not to exceed 10 days and 10,000 bbls of injected fluid. |
UIC | Form: (04 ) 401745514 9/4/2018 | Collect injection zone water samples from Navajo prior to injection of acid. |
Engineer | Form: (04 ) 402063300 6/4/2019 | 1)The Operator shall monitor the bradenhead pressure of the proposed well and all offset wells under Operator’s control which penetrate the injection target formation and have a treated interval separation of 300 feet or less. Monitoring shall occur from 24 hours prior to injection and shall continue until 24 hours after injection is complete. Recording shall be at a frequency of at least once per 24 hours with the capability of recording the maximum pressure observed during each 24 hour period. Operator shall notify COGCC Engineering staff if bradenhead pressures increase by more than 200 psig.
2)The casing must maintain an ability to maintain 300 psig of differential pressure. For example, if the injection pressure for well operations at 1,700 psig, the pressure test should be run at 2,000 psig. This is computed by starting at the injection pressure of 1,700 psig adding 300 psig (1700 psig + 300 psig = 2,000 psig) to compute a casing-tubing annulus test pressure.

UIC | Form: (04 ) 402063300 6/5/2019 | Results of Temperature and Radiologic Tracer tests will be reviewed by COGCC to determine whether the Navajo Formation has an adequate seal along 1st String Casing. Simply running the tests will not automatically fulfill the requirement for mechanical integrity. |
UIC | Form: (04 ) 402326307 2/28/2022 | This sundry submits a required 5-year water quality analysis. Next 5-year water quality analysis due by 02/27/2025. Submit next 5-year water quality analysis on a Form 43. |
