A bradenhead test submitted on a Form 17 is required prior to approval of the next years request for continued TA status. This approval is good for one year and a new application must be made before October 9, 2019 (1 year from prior application). Updates for the planned utilization of the well should be included with the application. A Form 5A is required to report all plugs currently downhole which isolate the perforated interval. The Form 7 reporting must reflect the well status dates reported on the Form 4-TA request.

This Oil and Gas Location is within a CPW mapped Elk Production area. The well work and pipeline installation activities should not take place from May 15 to June 30. If operations must take place from May 15 to June 30, operator will consult with CPW to determine site-specific measures to avoid, minimize, and mitigate adverse impacts to wildlife and the environment.
