2. Submit the gas and liquid sample analysis from previous Form 17 bradenhead test on a Form 43

For Wells that have sustained casing pressure above the action threshold, flows liquids or continuous gas from the bradenhead annulus as determined through diagnostic testing and evaluation, the Operator will provide a PMP with the Form 4, including a description of the Operator’s findings from the previous 6 months, number of times the well was blown down, a current WBD and future plans for more frequent monitoring or testing, pressure mitigation, or remedial well work by 4/29/2022
Include diagnostics on potential communication between BH and Intermediate annulus
4. Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges.
5. Complete a Form 17 Bradenhead test upon completion of diagnostic work |

2) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method.
