01/07/2026 | 1) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations.
2) Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.
3) If any pressure tests fail: Contact ECMC engineering prior to continuing operations.
4) All repair work shall be completed by 7/6/2026.
5) Submit a Form Form 5A, Completed Interval Report, within 30 days of work completion, to report the updated packer depth, if packer is set at different depth. |
