The sundry should include details of the future plans, flow information, number of times well was blown down, pressure data and a discussion of the abatement history for the previous 6 months. Please include the proposed pressure mitigation plan in the sundry. 2) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report 3) Submit a current WBD on a Form 4 Sundry “COMPLIANCE with CONDITION OF APPROVAL (COA)” by 03/31/23

Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. By 8/9/2025 , conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans and the flow rate information and pressure data. 3. Operator will implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up.

The sundry should include details of the new proposed mitigation plan due to continuous flow at the end of the test, bradenhead test description, and the flow rate information and pressure data.

1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. Operator will implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up. 3. If a sample has not been collected from surface and production casing within the last twelve months collect bradenhead gas samples for laboratory analysis. Sampling will comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results will be provided to the ECMC within three months of collecting the samples. 4. No later than 2/27/2027, conduct a bradenhead test, submit a Form 17 within ten days of the test. The well should be shut in for seven days to monitor and collect data to characterize build up pressures prior to conducting the bradenhead test. Within 60 days of conducting the bradenhead test, submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the new proposed mitigation plan due to continuous flow at the end of the test, bradenhead test description, and the flow rate information and SCADA pressure data.
