06/12/2018 | This approval is good for one year and a new application must be made before August 29, 2018(1 year from prior application). Updates for the planned utilization of the well should be included with the application.
A Form 5A is required to report all plugs currently downhole which isolate the perforated interval.
Form 7 reporting must reflect the well status dates reported on the TA request.

09/13/2018 | 1)Provide 48 hour notice of plugging MIRU via electronic Form 42.
2)Check bradenhead pressure and perform a bradenhead test prior to MIRU if the
pressure is greater than 50 psi. Submit results electronically on a Form 17.
3)Properly abandon flowlines as per Rule 1105. File electronic Form 42 once
abandonment complete. Within 30 days of an operator completing abandonment
requirements for an off-location flowline or crude oil transfer line the operator shall
submit a Flowline Report, Form 44.
4)Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.
5) If unable to circulate perf cluster at 379', move up hole and perf at 340' and try the same. |
