1) Provide 48 hour notice prior to spud via electronic Form 42. 2) If production casing is set provide cement coverage to at least 200’ above the shallowest completed interval and cement across Cheyenne/Dakota interval: (2550’-2000’ minimum). Run and submit CBL to verify cemented intervals. 3) If well is a dry hole set plugs at the following depths: 40 sks cement +/- 50’ above the Morrow, 40 sks cement +/- 50’ above the Cherokee, 40 sks cement +/- 50’ above any DST w/ oil/gas show, 40 sks cement at 2550’, 40 sks cement at 2000’, 50 sks cement from 50’ below surface casing shoe up into surface casing, 15 sks cement in top of surface casing, cut 4 ft below GL, weld on plate, 5 sks cement each in rat hole and mouse hole.

The intial production test period of 60 days begins and runs consecutively from the day the well is turned on or flowed back. An update providing gas production volumes, gas volumes used onsite and a gas analysis including H2S shall be submitted via Form 4, Sundry Notice prior to the conclusion of the 60 days to request continued flaring of gas.

1) The flaring of natural gas shall be allowed in accordance with COGCC Rule 912 for the initial production test period. The combustion device must have a design destruction efficiency of at least 98% for hydrocarbons. 2) Notify the local emergency dispatch or the local governmental designee of any natural gas flaring. 3) The operator is required to obtain and maintain any required air permits from CDPHE. 4) The metered flare volumes shall be reported by Form 7, Operator??s Monthly Report of Operations.

1) Provide 2 business day notice of plugging MIRU via electronic Form 42. 2) After placing the shallowest hydrocarbon isolating plug (6500'), operator must wait a sufficient time to confirm static conditions. If at any time after placing this plug there is evidence of pressure or of fluid migration, contact COGCC engineering before continuing operations. 3) Prior to placing the 605' plug: verify that all fluid migration (liquid and gas) has been eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer for an update to plugging orders. 4) After isolation has been verified, pump plug and displace. If cement is not circulated to surface, shut-in, WOC 4 hours then tag plug – must be at 455' or shallower and provide 15 sx plug at the surface. 5) Leave at least 100’ of cement in the wellbore for each plug. 6) Properly abandon flowlines as per Rule 1105. Pursuant to Rule 911.a. Closure of Oil and Gas Facilities, Operator will submit Site Investigation and Remediation Work plans via Form 27 for COGCC prior approval before cutting and capping the plugged well, conducting flowline abandonment, and removing production equipment. Pursuant to Rule 1105.f. Abandonment Verification, within 90 days of an operator completing abandonment requirements for a flowline or crude oil transfer line, an operator must submit a Field Operations Notice, Form 42-Abandonment of Flowlines for on-location flowlines, and a Flowline Report, Form 44, for off-location flowlines or crude oil transfer lines. 7) With the Form 6 SRA operator must provide written documentation, which positively affirms each COA has been addressed.

Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples. If there is a need for sampling, contact COGCC engineering for verification of plugging procedure.

An Open-hole Resistivity Log with Gamma Ray Log will be run from TD into the surface casing. A Cement Bond Log (CBL) with Gamma-Ray will be run on production casing, or on intermediate casing if a production liner is run. The Form 5, Completion Report, will list all logs run and have those logs attached.
