1) Provide 48 hour notice prior to spud via electronic Form 42. 2) If production casing is set provide cement coverage to at least 200’ above the Marmaton and cement Cheyenne/Dakota interval (4850’-4200’ minimum). Run and submit CBL to verify cemented intervals. 3) If well is a dry hole set plugs at the following depths: 40 sks cement +/- 50’ above the Morrow, 40 sks cement +/- 50’ above the Marmaton, 40 sks cement +/- 50’ above any DST w/ show, 40 sks cement at 4850’ up, 40 sks cement at 4200’ up, 50 sks cement from 50’ below surface casing shoe up into surface casing, 15 sks cement in top of surface casing, cut 4 ft below GL, weld on plate, 5 sks cement each in rat hole and mouse hole.

Plugging: Note changes to plugging procedure. 1) Provide 48 hour notice of MIRU via electronic Form 42 - Start of Plugging Operations. 2) After placing the shallowest hydrocarbon isolating plug, operator must wait a sufficient time to confirm static conditions. If at any time after placing this plug there is evidence of pressure or of fluid migration, contact COGCC engineering before continuing operations. 3) Shoe plug: Tag plug 50’ above surface casing shoe. 4) Surface plug: Cement from 50' to surface in casing and annulus. 5) With the Form 6 SRA operator must provide written documentation which positively affirms each COA from the approved Notice of Intent to Abandon has been addressed. Bradenhead: Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations. The Form 17 shall be submitted within 10 days of the test. 1) If before opening the Bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples. If there is a need for sampling, contact COGCC engineering for verification of plugging procedure. Venting during plugging: Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare. Flowlines: Properly abandon flowlines per Rule 1105. File electronic Form 42 once abandonment is complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line, the operator must submit a Flowline Report, Form 44.

Downing St. Englewood, CO 80113 303-789-1798 303-761-9067 (fax) Certification to Discharge Under CDPS General Permit COR-03000 Stormwater Discharges Associated with Construction Certification Number COR 39788 Pocket Money # 1 well T10S, R56W Sec. 36: SW1/4NE1/4 Lincoln County, Colorado Description of Well Site and Construction Area 1-4 acre site with access road for drilling of oil and gas well. Terrain is flat with native grass and sagebrush. No receiving/surface water will be affected by well pad or access road. Surface Roughening will be used on low side of drill site to reduce speed of runoff, increase infiltration, reduce erosion, trap sediment and prepare soil for reseeding. Mud and Water Pits will be reclaimed within 6 months weather permitting. Topsoil will be separated and spread on drill site as final operation before reseeding operations. To prevent soil erosion site will be disked and straw crimped in or manure spread. Stormwater Management Plan (SWMP) is on file in Wiepking-Fullerton Energy, L.L.C. office. Spill Prevention, Control and Countermeasure Plan is on file in Wiepking-Fullerton Energy, L.L.C. office.
