On a multi-well pad, these open hole logs are only required on one of the first wells drilled on the pad and the Drilling Completion Report - Form 5 for every well on the pad shall identify which well was logged.

1. Submit a Form 4 reporting the results of the operation within 30 days after the Pressure Blanket well has returned to production. Report the following: a) The total volume of gas or water injected in the well. b) The dates during which injection occurred. c) The total volume of gas or water recovered from the well during withdrawal. d) The dates during which withdrawal occurred e) The well’s shut in pressure after the Blanket fluid has been withdrawn. f) Pressure monitoring data during offset frac operations. 2. All injected and withdrawn gas or water must be reported on a Form 7.
