Approval #1).
(3) Operator shall provide cement coverage from the production casing shoe (i.e. 1st String) to a minimum of 500 feet into the Surface Casing as specified in the Casing section of this permit application. Verify production casing cement coverage with a cement bond log.
(4) Approval of this form does not authorize injection. Authorization to inject requires approval of Form 31 and
Form 33.
(5) Before stimulating the objective injection formation, collect water sample and analyze for Total Dissolved Solids.

| UIC | Form: (02) 400850708 03/09/2016 | 1.Prior approval of Form 4 is required for step rate and injectivity tests. Operator is required to contact COGCC to discuss Step Rate Test or Injectivity Test criteria for Maximum Surface Injection Pressure determination.
2.FOR ALL NEW DRILL AND CONVERTED UNDERGROUND INJECTION CONTROL WELLS a Cement Bond Log (CBL) IS REQUIRED on the cased portions of the hole from the bottom of the casing string to the top of the next shallower casing string for all casing strings other than the Surface Casing. Only a PDF, TIFF, or PDS visual image is required.
3.Operator must provide all tops of formations encountered from surface to TD on the Form 5 when submitted.

| Engineer | Form: (04) 401120880 09/29/2016 | 1) Notify Bob Koehler at (970) 894-2100 x 5147 at least 24-hours prior to commencing the stimulation.
2) The volume must not exceed 26,000 bbls and the stimulation duration must not exceed 26 days.
3) Contact Bob Koehler at (970) 894-2100 x 5147 to discuss test progress when the cumulative volume injected reaches 15,000 bbls. Pending results, COGCC Staff may consider an increase in the allowable test volume and/or duration.
4) Monitor the bradenhead, tubing and casing pressures during injection of the entire approved 26,000 bbl volume. Notify Bob Koehler if bradenhead pressure increase of more than 100 psi is observed. |
| UIC | Form: (04) 401120880 09/29/2016 | Operator to file a Frac Focus Report upon conclusion of stimulation project. Report due 30 days after rig pulls off site. |
| UIC | Form: (04) 401120880 09/29/2016 | Operator to submit a summary report to COGCC via Form 4-Sundry Notice at conclusion of project. Include brief discussion of operations, any successes, failures, and summary plots, graphs, and/or data as needed. COGCC wants general idea of injection pressures, injection rates, sand volumes. |
| UIC | Form: (04) 401165718 06/06/2017 | Both fracture height and length are to be monitored according to the proposal. (Discussed height of fractures with Garrett Elsener at Caerus. He believes there is little likelyhood that fractures will leave the Ohio Creek because the top perforation will be at 5405 ft and the top of the Ohio Creek is at 5124 ft (181 ft). Furthermore the there is about 150 ft of shaley material (based on Gamma Ray-Run #1 from Cased Hole log) above proposed perforations. Mr. Elsener also does not believe that the fractures will extend beyond 1/4-mile from the well. |
| UIC | Form: (04) 401337831 07/13/2017 | Operator is granted permission for 3 week extension to complete project. Extension through the end of August 4, 2017. Additional week granted after discussion with operator. Object is to make sure project is fully completed and a second sundry is not needed. Original 200,000 bbl maximum treatment volume specified in Sundry # 401165718 remains in effect. |
