| Engineer | Form: (04) 403217973 12/07/2022 | 1) At the conclusion of the 6 months from BH test date (3/14/23), submit a Form 4 BHP Sundry that summarizes current well condition. The sundry should include details of the future plans, flow information, number of times well was blown down, pressure data and a discussion of the abatement history for the previous 6 months.
2) Operator shall comply with the Notice to Operators Drilling Wells in the Buzzard, Mamm Creek, and Rulison fields, Garfield County and Mesa County – Procedures and Submittal Requirements for Compliance with COGCC Order Nos. 1-107, 139-56, 191- 22, and 369-2 (“Bradenhead NTO”), dated July 8, 2010.
3) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report

2) Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges.
3) Complete installation of BH monitoring system as soon as parts are available
