In the event that visual monitoring and observation of the surface water features indicate anomalies; or that samples collected from monitoring wells (including sentinel and down gradient monitoring wells) indicate increasing concentrations of BTEX, Encana shall institute a contingency plan. This plan may include increased sampling frequency of surface water and monitoring wells and potential activating the air sparging system. Other contingency plans may be warranted.

1) Prior to initiation of plugging operations, a bradenhead test shall be performed. If any pressure remains at the conclusion of the test or any liquids were present call COGCC Engineer for sampling requirements and changes to the plugging procedure. Form 17 shall be submitted within 10 days of the test. 2) Provide 48 hour notice of plugging MIRU via electronic Form 42. 3) Properly abandon flowlines per Rule 1103. File electronic Form 42 when flowline abandonment is complete.

Based on review of the information provided, it appears that no further monitoring is necessary at this time and COGCC approves the abandonment of monitoring wells (MW-2, MW-4, MW-8, MW-12, MW-17, and MW-22) in accordance to the Division of Water Resources guidelines. REM 1815 shall remain open until the Operator demonstrates that the monitoring wells have properly been abandoned.

Based on a review of the information provided, it appears that no further action is necessary at this time and COGCC approves the closure request. Should conditions at the site indicate contaminant concentrations in soils exceeding COGCC standards, or, if surface or groundwater is found to be significantly impacted, further investigation and/or remediation activities may be required at the site.
