12/14/2018 | 1)Provide 48 hour notice of plugging MIRU via electronic Form 42.
2)Check bradenhead pressure and perform a bradenhead test prior to MIRU if the
pressure is greater than 50 psi. Submit results electronically on a Form 17 and contact COGCC Area Engineer.
3)Properly abandon flowlines as per Rule 1105. File electronic Form 42 once
abandonment complete. Within 30 days of an operator completing abandonment
requirements for an off-location flowline or crude oil transfer line the operator shall
submit a Flowline Report, Form 44.
4)This well has federal minerals. Operator shall notify COGCC engineering staff of any
plugging changes required by the BLM or unexpected conditions in the field as soon as
feasible.
5)Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.
6)Ensure DV tool set at 2269' in the production casing (4.5") is covered with at least 50' of cement both above and below.
