Verify coverage with cement bond log. 3) Comply with Rule 321. Run and submit Directional Survey from TD to base of surface casing. Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

1) The flaring of natural gas shall be allowed in accordance with COGCC Rule 912 for the initial production test period. 2) Notify the local emergency dispatch or the local governmental designee of any natural gas flaring. 3) Submit updated requests to flare to COGCC on Form 4 (Sundry Notice), which discuss whether or not any new gathering systems or increased development of the field have changed pipeline economics such that installation of a new pipeline would be an economically viable option in lieu of continued flaring. 4) The operator is required to obtain and maintain any required air permits from CDPHE. 5) The flare volumes shall be reported by Form 7, Operator’s Monthly Report of Operations.

1) Flaring of natural gas shall comply with COGCC Rule 912. 2) Submit requests to flare annually to COGCC on Form 4 (Sundry Notice), which discusses timing to connect to sales line, plans to develop infrastructure, and justification for continued venting/flaring. Provide gas analysis including H2S with each Sundry Notice.

NOTE: Changes in plugging procedure - surface shoe plug shall be perforated at 1287' and 50 sx pumped, this plug needs to be tagged at 1187' or shallower. 1) Provide 48 hour notice of plugging MIRU via electronic Form 42. 2) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.

Prior to starting plugging operations a bradenhead test shall be performed. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. The Form 17 shall be submitted within 10 days of the test.
