Material Handling - Cuttings Management Operations: The following COA will apply: • All materials brought to this location that exceed the requirements in Table 910-1 will be placed in an area of the site that is completely segregated from materials that meet the requirements in Table 910-1. This area must be bermed and sufficiently impervious; with appropriate BMPs being in place during operations at the dry cuttings pit location. Sufficient stormwater BMPs must be implemented to prevent any stormwater run-on and /or stormwater run-off.

Drilling/Completions: The following COA will apply: • The moisture content of water/bentonite based mud (WBM) generated drill cuttings managed onsite shall be kept as low as practicable to prevent accumulation of liquids greater than de minimis amounts. After drilling and completion operations have been completed, all WBM drill cuttings that will remain on the Clough RWF 44-18 Well Pad location must be sampled and meet the applicable standards of Table 910-1. No liners (if used) are allowed to be disposed of with the drill cuttings. The operator will implement measures (covers, misting, etc.) in trucks to reduce dust and PM emissions during transport of WBM solids and cuttings materials from the well pad DOE 1-M-18 Well Pad (Location ID #335744) to the repurposed Clough RWF 44-18 Well Pad (Location ID #335146) where the dry cuttings trench / drilling pit is located. Commercial disposal of drill cuttings (if necessary) will require notification (no prior approval) to COGCC via a Form 4 Sundry Notice.

Construction: The following COAs will apply: • Secondary containment for any area on the pad where cuttings will be stored/treated/amended must be constructed to be sufficiently impervious to contain any accumulations of fluids. The cuttings initial staging area and the dry cuttings trench / drilling pit portion of the pad where cuttings will be either temporarily stored or disposed of must be bermed. • Since this well pad location will be amended and expanded as needed; operator shall submit a scaled as-built drawing (plan and cross-sectional views) of this location, showing the dry cuttings trench / drilling pit, containment features, and stormwater features within 30 calendar days of conclusion of construction. • The access road between the Clough RWF 44-18 Well Pad (Location ID #335146) and the DOE 1-M-18 Well Pad (Location ID #335744) will be maintained as to not allow sediment to migrate from the access road to nearby surface water or any drainages leading to surface water. • Strategically apply fugitive dust control measures to the access road between the Clough RWF 44-18 Well Pad (Location ID #335146) and the DOE 1-M-18 Well Pad (Location ID #335744) to reduce fugitive dust and coating of vegetation and deposition in water sources. Throughout all phases of operations, operator shall employ practices for control of fugitive dust, including, but not limited to, the use of water and other dust suppressants (used as required, dependent upon the level of activity, moisture conditions, etc.), use of speed restrictions, regular road maintenance, or restriction of construction activity during high wind days. Additional management practices such as wind breaks and barriers may be used. • During the construction phase, operator shall ensure that truckloads of dirt, sand, aggregate materials, and similar materials used for the re-construction of the access road and well pad are covered to reduce dust and PM emissions during transport. • Operator shall submit a scaled as-built drawing (plan and cross-sectional views) of this location, showing where the cuttings from the DOE 1-M-18 Well Pad (Location ID #335744) have been placed, total depth of cuttings, and thickness of natural soil cover, within 30 calendar days of conclusion of cuttings placement and prior to reclamation.

Planning: The following COAs will apply: • Only drill cuttings generated from the DOE 1-M-18 Well Pad (Location ID #335744) shall be placed in the Clough RWF 44-18 Well Pad (Location ID #335146) dry cuttings trench / drilling pit. No other E&P waste nor drill cuttings from other locations shall be transported, managed, and disposed of at the Clough RWF 44-18 Well Pad (Location ID #335146) dry cuttings trench / drilling pit. • Provide Notices as described in the most current version of the Northwest Notification Policy. • Provide 48 hour notice prior to start of cuttings transfer from the DOE 1-M-18 Well Pad (Location ID #335744). • Provide 24 hour notice after completion of cuttings transfer from the DOE 1-M-18 Well Pad (Location ID #335744). • Provide 48 hour notice prior to start of native soil cover placement over the cuttings. • Within 45 days of completion of the management of cuttings generated from the DOE 1-M-18 Well Pad (Location ID #335744), the operator shall provide a report detailing the API numbers, the volume of cuttings generated, and the actual cuttings management process that was performed (including sampling activities and analytical results).
