Cuttings Mangement Operations: The following COAs will apply: • All materials brought to this location that exceed the requirements in Table 910-1 will be placed in an area of the site that is completely segregated from materials that meet the requirements in Table 910-1. This area must be bermed and sufficiently impervious; with appropriate BMPs being in place during operations at the dry cuttings pit location. Sufficient stormwater BMPs must be implemented to prevent any stormwater runon and /or stormwater runoff. • The moisture content of any drill cuttings placed in the dry cuttings pit or the cut portion of the well pad shall be as low as practicable to prevent accumulation of liquids greater than de minimis amounts.

Construction: The following COAs will apply: • Secondary containment for any area on the pad where cuttings will be stored/treated/amended must be constructed to be sufficiently impervious to contain any accumulations of fluids. The cuttings initial staging area and the cut slope portion of the pad where cuttings will be either temporarily stored or used to reduce the cutslope must be bermed. • Since this well pad location will be amended and expanded as needed; operator shall submit a scaled as-built drawing (plan and cross-sectional views) of this location, showing dry cuttings pit, containment features, and stormwater features, within 30 calendar days of conclusion of construction. • Operator shall submit a scaled as-built drawing (plan and cross-sectional views) of this location, showing where the cuttings from each well pad have been placed, total depth of cuttings, and thickness of natural soil cover, within 30 calendar days of conclusion of cuttings placement and prior to reclamation.

Planning: The following COAs will apply: • Provide Notices as described in the most current version of the Northwest Notification Policy. • Provide 48 hour notice prior to start of cuttings transfer from the Federal MV 60-8D Pad aand the Puckett GV 1-8 Pad. • Provide 24 hour notice after completion of cuttings transfer from the Federal MV 60-8D Pad aand the Puckett GV 1-8 Pad. • Provide 48 hour notice prior to start of native soil cover placement over the cuttings.

Only drill cuttings generated from the MV 60-9D (Location ID 334950) and the Puckett GV1-8 (Location ID 323823) shall be placed in the Federal GM 41-8 cuttings trench (Location ID 334794). No other E&P waste nor drill cuttings from other locations shall be transported, managed, and disposed of at the Federal GM 41-8 cuttings trench (Location ID 334794).

- Utilize staked soil retention blankets for erosion control and reclamation of large surface areas with 1.5:1 or steeper slopes. Avoid use of plastic blanket materials. - Use only certified weed-free native seed in seed mixes, except for non-native plants that benefit wildlife - TEP will use certified, weed free grass hay, straw, hay or other mulch materials used for the reseeding and reclamation of disturbed areas.
