Operator required to update Area of Review or Offset Well Evaluation. Original Evaluation did not contain plugged and abandoned or drilled and abandoned wells within 1/4-mile of Emerald 79X. This will include submitting updated Map of all Oil and Gas Wells Within 1/4-Mile of the Emerald 79X and a list of those wells.

1. Injection is not authorized until approval of Subsequent Forms 31 and 33. 2. If retrieving water sample(s) from injection zone(s) for analysis do so before stimulating formation. Samples must be analyzed for Total Dissolved Solids at a minimum. 3. PRIOR TO PERFORMING OPERATIONS: Operator is required to contact COGCC to discuss Step Rate Test or Injectivity Test criteria for Maximum Surface Injection Pressure determination. Prior approval of Form 4 is required for step rate and injectivity tests. 4. PRIOR TO PERFORMING OPERATIONS: Approval of Form 4 is required for acid and fracturing jobs. (New as of 4/13/2016). 5. Operator must provide all tops of formations encountered from surface to TD on the Sundry Notice Form 4.

Based on precedents from existing Navajo disposal wells operator required to notify Surface Owner and Mineral Owner of intension to convert Emerald 79X into disposal well for injection of salt water, produced water, and/or approved E&P fluids into Navajo Formation. Existing 1957 Surface Use Agreement (Doc # 1801184) is for Unitized Enhanced Recovery in Weber Sandstone.

Within thirty (30) days of initial injection, Ursa Operating shall submit a Form 5A Completed Interval Report to notify the COGCC of the date of initial injection and update the well status.

In accordance with COGCC Rule 326.a.(5), Mechanical Integrity Tests (“MITs”) shall be performed at 5-year intervals on the Emerald 79X well. The first 5-year period shall commence on the date the initial mechanical integrity test is performed. An MIT is also required after resetting the tubing or packer whenever the tubing or packer are disturbed during workover operations. All injection well MITs must be witnessed by COGCC Staff.

The initial Maximum Daily Injection Rate is limited to 10,000 barrels/day. If operator wishes to inject at a rate higher than 10,000 barrels/day they shall install and maintain a seismic monitoring station. Operator shall consult with COGCC to discuss location and design of the station and make an application via Form 4-Sundry Notice.

The nature of the injected fluids shall be monitored with sufficient frequency to yield data representative of their characteristics. Therefore, as a condition of approval a water analysis of fluids injected into the Emerald 79X is required within one year of the commencement of injection. The injected water must be analyzed for total dissolved solids (“TDS”) and major cations and anions. This analytical data set shall be submitted to COGCC via a Form 4-Sundry Notice. COGCC requests, but does not require, submittal via COGCC’s Electronic Data Deliverable (“EDD”) Format. Instructions for submitting EDD laboratory reports can be found on the COGCC website under: HELP/Submitting Laboratory Analytical Data Electronically. Additional analyses of the fluids injected into the Emerald 79X well, as described above, are required at 5-year intervals. The first 5-year interval begins with the 1-year sample discussed in the preceding paragraph. Five year analytical results are to be submitted by Sundry Notice and and may be submitted via EDD.
