Note changes: Increased surface casing depth or require stage cement of Upper Pierre Sandy interval (Pawnee aquifer). 1) Provide 48 hour notice of spud via electronic Form 42. 2) Set at least 1750’ of surface casing or set at minimum of 760’ of surface casing and stage cement from 1750’ to 50’ above surface casing (due to 1630' WW in SESW10-9N-59W and 1560' base of U. Prre sandy zone on offset 123-07350 Ind. Log). Cement surface casing to surface. 3) Provide cement coverage to a minimum of 200' above Niobrara. Verify coverage with cement bond log. 4) Run and submit Directional Survey from TD to base of surface casing. Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

Open hole resistivity and gamma logs shall be run to describe the stratigraphy of the entire well bore and to adequately verify the setting depth of surface casing and aquifer coverage. On a multi-well pad, these open hole logs are only required on one of the first wells drilled on the pad and the Drilling Completion Report - Form 5 for every well on the pad shall identify which well was logged.

Well is in a CPW mapped Mule Deer Severe Winter Range & Mule Deer Winer Concentration High Priority Habitat. Although plugging and abandonment operations with heavy equipment will be allowed, the Operator is strongly encouraged to avoid them between December 1 through April 30.

Plugging 1) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations and electronic Form 42 Notice of Plugging Operations 48 hours prior to mobilizing for plugging operations.   2) Plugs and squeezes will be placed as stated in the Plugging Procedure section of the approved NOIA unless revised by COA or prior approval from ECMC is obtained. 3) The wellbore must be static prior to placing cement plugs which are to be a minimum of 100' in length for all but surface plugs. Mechanical isolation requires a 25' cement plug, minimum. For plugs not specified to be tagged, a tag is required if circulation is not maintained while pumping plug and displacing to depth. Tag at tops specified or shallower. Notify ECMC Area Engineer before adding cement to previous plug. 4) Place a 50' plug (minimum) at the surface, both inside the inner most casing and in all annular spaces. Surface plugs shall be circulated to surface. Document cement to surface in all strings during cut and cap. 5) With the Form 6 SRA operator must provide written documentation which positively affirms each COA has been addressed. 6) Operator must wait a sufficient time on all plugs to achieve the intended design. If at any time during the plugging there is evidence of previously unreported pressure or fluid migration, contact ECMC Area Engineer before continuing operations. 7) Plugging procedure as follows, Plug #1 - 5774-5574', CIBP with 40 sx of cement, NOTE: change to volume, Plug #2 - 2550-2450', 25 sx cement casing plug, Run a CBL 2400-0' to determine TOC, if cement top is deeper than 750' and shallower than 1699', skip Plug #3 and proceed to Plug #4, Plug #3 - 1850-0', 360 sx cement csg plug, skip Plug #4 and proceed to Plug #5, Plug #4 - 750', perf and circulate 260 sx of cement to the surface, WOC and tag. Use caution to only perf the intermediate casing, Plug #5 - 50' of cement at the surface in both the casing and the annulus per COA #4. 8) Submit an updated WBD and plugging procedure that correspond to the approved 6(N) (if needed) prior to filing Form 42 Notifications required in COA #1 above.

Properly abandon flowlines per Rule 1105. If flowlines will be abandoned in place, include with the Form 27: pressure test results conducted in the prior 12 months as well as identification of any document numbers for a ECMC Spill/Release Report, Form 19, associated with the abandoned line.

Consistent with Rule 911.a, a Form 27 must be approved prior to cut and cap, conducting flowline abandonment, or removing production equipment. Allow 30 days for Director review of the Form 27; include the Form 27 document number on the Form 44 for offsite flowline abandonment (if applicable) and on the Form 6 Subsequent.

Bradenhead Testing Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the ECMC within three (3) months of collecting the samples. If there is a need for sampling, contact ECMC engineering for verification of plugging procedure.

Operator will implement interim reclamation measures immediately after construction of the location to stabilize cut and fill slopes, stabilize the topsoil and spoil stockpiles, establish desirable vegetation and control weeds on the location. Jet pump used at the initial start-up of the well then replaced with pump jack.
