3) Run and submit Directional Survey from TD to base of surface casing. The operator shall comply with Rule 321, and it shall be the operator’s responsibility to ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

On a multi-well pad, these open hole logs are only required on one of the first wells drilled on the pad and the Drilling Completion Report - Form 5 for every well on the pad shall identify which well was logged. |

Due to the removal of surface facilities and sales lines, this well is considered Temporarily Abandoned. All other temporary abandonment requirements remain in place including a TA sundry, a 5A, and Form 7 reporting. Furthermore, all Inactive well requirements remain in full effect. |
| Engineer | Form: (04) 404145946 04/11/2025 | 1) Well last produced January 2025, and has been designated as Inactive. Within 6 months of a Well becoming Inactive action must be taken. No later than January 2026, this well must:
A. Be plugged and Abandoned,
B. Return to production so that it is no longer an Inactive Well,
C. Have an approved Form 5B, Inactive Well Notice,
D. Be designated as Out of Service by submitting a Form 6A, Out of Service Designation.
