Verify coverage with cement bond log. 3) Run and submit Directional Survey from TD to base of surface casing. Form 17, Doc #402262358, from 12/10/19 reported bradenhead pressure of 60 psi that blew down to a whisper and that samples were taken. To date the analytical results are not appearing in the COGCC database, please submit results by 7/22/2020.

1. Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. Prior to starting bradenhead mitigation, if a sample has not been collected within the last twelve months collect bradenhead and production gas samples for laboratory analysis. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results shall be provided to the COGCC within three months of collecting the samples. 3. Operator shall implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up. This mitigation plan may be used for six consecutive months from the test date reported on the Form 17. 4. At the conclusion of the six months (6/10/2020), conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans and the flow rate information and pressure data. Shut in bradenhead pressure shall not exceed 50 psig.

Bradenhead pressure mitigation via production facilities planned for Q3 2021. The results of this mitigation to date as well as the annual bradenhead test and gas sample analysis for 2022 are due by 12/31/22. The results should be summited via sundry and are to include volumes, rates and pressures maintained on the bradenhead in addition to a summary of the monthly monitoring data.

Stormwater management plans (SWMP) are in place to address construction, drilling and operations associated with oil and gas development throughout the State of Colorado. BMPs will be constructed as necessary to prevent stormwater from leaving the construction site. BMPs used will vary according to the location, and will remain until the pad is reclaimed

Spill Prevention Control and Countermeasures (SPCC) plans are in place to address any possible spill associated with oil and gas operations throughout the State of Colorado. • Materials and fluids will be stored in a neat and orderly fashion. • Waste will be collected regularly and disposed of at an offsite facility. • Prompt cleanup is required of spills to minimize waste materials entering the stormwater runoff. • Drip pans will be used during fueling and maintenance to contain spills or leaks. • Cleanup of trash and discarded material will be done at the end of the work day. • Cleanup will consist of monitoring the road, location and any other work areas. • Material to be cleaned up includes trash, scrap, and contaminated soil.
