Verify coverage with cement bond log. 3) Run and submit Directional Survey from TD to base of surface casing. Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

1) The flaring of natural gas shall be allowed in accordance with COGCC Rule 912 for the initial production test period. 2) Notify the local emergency dispatch or the local governmental designee of any natural gas flaring. 3) Submit updated (requests to flare) to COGCC on Form 4’s (Sundry Notices), which discuss whether or not any new gathering systems or increased development of the field have changed pipeline economics such that installation of a new pipeline would be an economically viable option in lieu of continued flaring. 4) The operator is required to obtain and maintain any required air permits from CDPHE. 5)The flare volumes shall be reported by Form 7.

Operator will participate in the Colorado Oil & Gas Association Voluntary Baseline Groundwater Quality Sampling Program. Operator will implement interim reclamation measures immediately after construction of the location to stabilize cut and fill slopes, stabilize the topsoil and spoil stockpiles, establish desirable vegetation and control weeds on the location.
