Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

1.Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. Prior to starting bradenhead mitigation, if a sample has not been collected within the last twelve months collect bradenhead and production gas samples for laboratory analysis. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results shall be provided to the COGCC within three months of collecting the samples. 3. During the first day of mitigation operator shall implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up. This mitigation plan may be used for six consecutive months. 4. At the conclusion of the six months, conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans and the flow rate information and pressure data.

According to correspondance with the Operator, the initial pressure seen on the 9/27/19 Bradenhead test did not return and therefore the mitigation plan submitted and approved 12/31/2019 was not required. A followup sundry was due in 6/2020 per COA. Based on the Bradenhead test of 12/7/2020 the Operator may return this well to monthly monitoring and annual Bradenhead testing.

Plugging 1) Provide 48 hour notice of plugging MIRU via electronic Form 42. 2) Contact COGCC Area Inspector prior to commencing plugging operations. 3) Plugs and squeezes will be placed as stated in the Plugging Procedure section of the approved NOIA unless revised by COA or prior approval from COGCC is obtained. 4) The wellbore must be static prior to placing cement plugs which are to be a minimum of 100' in length for all but surface plugs. Mechanical isolation requires a 25' cement plug, minimum. For plugs not specified to be tagged, a tag is required if circulation is not maintained while pumping plug and displacing to depth. Tag at tops specified or shallower. Notify COGCC Area Engineer before adding cement to previous plug. 5) Place a 50' plug (minimum) at the surface, both inside the inner most casing and in all annular spaces. Surface plugs shall be circulated to surface. Confirm cement to surface in all strings during cut and cap. 6) With the Form 6 SRA operator must provide written documentation which positively affirms each COA has been addressed. 7) Properly abandon flowlines as per Rule 1105. Pursuant to Rule 911.a. Closure of Oil and Gas Facilities, Operator will submit Site Investigation and Remediation Workplans via Form 27 for COGCC prior approval before cutting and capping the plugged well, conducting flowline abandonment, and removing production equipment. Pursuant to Rule 1105.f. Abandonment Verification, within 90 days of an operator completing abandonment requirements for a flowline or crude oil transfer line, an operator must submit a Field Operations Notice, Form 42-Abandonment of Flowlines for on-location flowlines, and a Flowline Report, Form 44, for off-location flowlines or crude oil transfer lines. 8) No current Form 17 on file with COGCC. Contact COGCC area engineer with results of pre-plugging bradenhead test for confirmation of plugging procedure prior to commencing plugging operations. 9) After placing the shallowest hydrocarbon isolating plug (5509-5274'), operator must wait a sufficient time on all subsequent plugs to confirm static conditions. If at any time after placing this plug there is evidence of pressure or of fluid migration, contact COGCC Area Engineer before continuing operations. 10) Plugging procedure is as follows, Plug #1 - 5896', CIBP with 2 sx of cement. Plug #2 - 5509-5274', 35 sx casing plug across the liner top. Plug #3 - 3035-2900', 25 sx casing plug. Plug #4 - 2000', perf and squeeze 50 sx with CICR at 1950'. Wait on cement 8 hrs and check for pressure or flow in the bradenhead space. Notify COGCC Area Engineer to confirm plugging orders. Plug #5 - 730', perf and squeeze 291 sx. Intention is for cement to circulate to surface inside and out and remain there. If well does not circulate but will take fluid without breaking down the shoe, squeeze 50 sx, leave 100 ft in the casing and move up to 50' below the shoe and try again. Notify COGCC Area Engineer to confirm plugging orders. Plug #5 - Surface plug - 50' of cement per instructions in COA #5.

Bradenhead Testing Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples. If there is a need for sampling, contact COGCC engineering for verification of plugging procedure.
